UMBC’s Financial Services – Student Business Services (SBS) have tailored UMBC’s forms specifically for the FCBIS event at UMBC: Now coded “CNMS-FCBIS”. When the Financial Services (SBS) receive your contributions, they will notify our financial manager.
Invoice Requests:
- Contributors complete the Sections 2, 3 and 4 on the UMBC’s OFF-CAMPUS-BILLING-REQUEST-FORM-CNMS-FCBIS.
- Email it to the address at the top of the form (ofinvoices@umbc.edu). Note that the ‘Homepage’ screen for the event (i.e., page 2 in the downloaded form) should be included with the submitted form.
- You will then receive an invoice directly from the UMBC Office.
- If the invoice is going to get paid by a credit card or a check, please follow the instruction below.
Credit Card Payments:
- Contributors need to complete UMBC’s CREDIT-CARD-AUTHORIZATION-FORM-CNMS-FCBIS.
- Email it to both addresses at the bottom of the form: Sandi Blasetti (sblase1@umbc.edu) and Amita Naik (anaik@umbc.edu)
- Importantly, please include the CNMS-FCBIS and Invoice # (if requested) in the memo.
